Student subscribers enrolling with Delta Dental for the first time will receive a separate ID card in the mail from Delta Dental. Students previously enrolled with Delta Dental for the prior plan year should have received an ID card at the time of enrollment and will not receive a new ID card for this plan year.
The ID card contains a Delta assigned ID number, referred to as an Alternate ID, that should be shared with your dentist at your next visit. Participating network dentists will submit claims to Delta Dental of Indiana using the Delta assigned ID number.
Members are required to provide their Alternate ID, (the Delta assigned ID number) or the Member ID associated with your AHP Account when contacting Delta Dental customer service and when utilizing the Member Portal and other online tools.
If you need your Dental Member ID Number before you get your ID card:
Your Delta Dental Member ID number is derived by your United Healthcare Member (UHCSR ID) number. Your UHCSR is a 7 digit number (example 1241234). To get your Delta Member number, add two number eight (8’s) to the beginning of the number.
UHCSR ID 1241234 – Dental ID will be 881241234
UHCSR ID 8841234 – Dental ID will be 888841234
If for some reason you did not receive an ID card or no longer have your dental ID card from last year, or you need to request an additional ID card please contact Delta Dental customer service at 800-524-0149. You may also order an ID card online through www.memberportal.com. *Please note, you must have your Delta assigned alternate ID number or Member ID number available when contacting customer service.
